Camp Registration Terms & Conditions
I do hereby give permission to YMCA Camp Miller to transport the child named off the camp property for the purpose of medical care, program activities, and to participate in all camp activities deemed appropriate by the Director. In the event I cannot be reached in an emergency, I hereby give permission to the physician selected by the camp to hospitalize, secure proper treatment for, and to order injection, anesthetic or surgery for the child named. YMCA Camp Miller has my permission to use photographs taken of my child while at camp for promotional purposes. No refunds are given when camper goes home early in cases of disciplinary action or homesickness. I will notify the camp if my child has any serious restrictions related to his/her participation in the camp program. I further understand that the camp will not carry health insurance on my child. I have indicated my insurance company's name and group/policy numbers in this application.

The health history in this application is correct and complete to the best of my knowledge. I hereby give permission to the camp personnel to provide routine health care for the participant. In case of a life or death emergency, I hereby give permission to the physician/facility selected by the camp personnel to hospitalize, secure proper treatment for, and order injection, anesthesia or surgery for the participant in the event that the participant cannot make the decision on his or her own. The camp and its employed personnel shall be relieved of any responsibility. This completed form may be photocopied for trips off of the camp property.

Job Application Terms & Conditions
I authorize investigation of all statements herein, including any checks of criminal records, and release the camp and all others from liability in connection with same. I understand that, if employed, I will be an at-will employee unless there is an agreement or law which alters that status. Furthermore, I understand that misrepresentations or falsifications herein or in other documents completed or submitted by the applicant will result in dismissal, regardless of the date of discovery by the camp.

Personal Information
Any information you submit through the YMCA Camp Miller web site will be held in the strictest confidence. In no way will your personal information ever be used without your consent. To ensure this, all forms on our web sites that send information to a database that may be used in the future, e.g. mailing lists, registration, etc., have check boxes where you specify whether or not you would like to receive information from us in the future. Forms used just to send email, post messages, etc., do not include the option, but will never be used to send information. Be assured that we do not sell our name and address lists. If you choose to receive information from us, each message will also include instructions on how to "unsubscribe" from future mailings should you wish to cancel. The only information we automatically collect is that related to the use of our web site. This information is anonymous. The information is very generic, such as the number of visits our web site receives, the types of browsers used and the number of files downloaded, but never personal information. This information allows us to create better content aimed at our viewer's preferences.

How does SSL guarantee your information is kept safe?
